The National Board of Health and Welfare (NBHW) has tasked Umeå University with coordinating the design of a national, standardised assessment test for physicians with a non-EU/EEA medical license. The test of theoretical and practical knowledge is expected to enter use in October.

The task is part of the NBHW:s ongoing work with shortening the delays for assessment of foreign medical educations. The task for Umeå University also includes the creation of a national reference group, where representatives from Sweden's medical faculties and the NBHW will take part and the proposed design of the test will be discussed.

- The reference group will ensure that the structure of the test is good and that the test can be given in a qualitative manner. It is important that we gather the faculties and try to achieve a unified vision on the contents and difficulty level for a national test of physicians. The plan is that the teachers at the medical faculty here in Umeå creates the questions for the theoretical part and the stations for the practical part, says Magnus Hultin, university lector at Umeå University's Institution for Surgical and Perioperative Science, who will be responsible for / in charge of the test.

The test will replace the TULE test, which is offered to those who have a non-EU/EEA medical degree, and Magnus Hultin says that one of the advantages is that theoretical and practical knowledge are emphasised equally.

- There will be a transparent process in regards to how (theoretical) questions and practical tasks are designed and validated. The goal is to create a knowledge test with strong validity and reliability for both theoretical and practical aspects, says Magnus Hultin.

According to Magnus Hultin, the theoretical part is supposed to be offered in Swedish, with some parts in English, and a passing score on the theoretical part will be required for progressing to the practical part. After completing the test, a course in laws and regulations and a short internship follow. After completing those, the physician can apply for a Swedish medical license. The length of the internship has not yet been decided. In May the NBHW will announce further details on the internship and which rules will apply for those who have already begun the licensing process under the old system.

The tentative plan is for the knowledge test to be launched in October and the first round of the theoretical part will be held in Umeå and one additional site, currently undecided. Gothenburg has entered an agreement to be able to offer the practical test there in the future.

The agreement between Umeå University and the NBHW lasts until December 31st in 2017, after which three more years may be added.

The National Board of Health and Welfare (NBHW) recently tasked Umeå University with developing the new tests. The current TULE test for foreign medical graduates tests their knowledge up to graduation level, after which the foreign medical graduates have to complete the 18-month AT service to get their licenses. However, many have gotten stuck in the process because of a shortage of AT positions.

The new tests will instead measure the physician's knowledge up to license level, with AT service-level knowledge included. This is a way to reduce not only the demand for AT positions but also the need for documentation from the country of education, says Erik Magnusson at the NBHW.

– For refugees, it is a matter of luck whether they get to bring their documents, which means that people with the same abilities currently face different levels of difficulty in getting their licenses, says Erik Magnusson.

The Swedish Medical Association (SMA) welcomes the new tests but also calls for better cooperation between universities, employment services and county councils, so that the physicians will get the proper conditions for taking the test, otherwise it could become more of an advanced test of the Swedish language rather than medical knowledge.

– It is important that the test measures the actual knowledge and not how good your opportunities to learn Swedish or experience Swedish health care have been, says Sofia Rydgren Stale, chairman of the SMA committee for education and research.

The knowledge test will, according to Magnus Hultin at Umeå University, be given in two parts - theory and practice. The theoretical part will probably be similar to the Swedish SAT (SweSAT; högskoleprovet) in its design and can be done in a day. After passing it, the physician will be allowed to take the practical test according to a random sample principle. It may involve fictional healthcare situations with actors as patients where teachers of the medical degree education are present.

After completing the knowledge test, the foreign physician will do an internship for six months. The test will be done in Swedish.

I have been to the KIT tule site and it said Umeå universitet handles the tule program for höst 2016. Is there any further news and information you have on the subject?

Hello Nils. First I am grateful for the website. My question is' do you think it will be more difficult exam? And although it may be difficult? Previous TULE exams include only 5 lessons(obstetric, pediatric, psychiatry, medicine, surgery). Do you think also new TULE exam includes only these lessons? Or much?

Hello doktor_bu_nee,
The new test will be more challenging because it will be testing the full knowledge of the applicant up to the Swedish licensing level, which goes beyond the graduation level tested by TULE.

Some additional information:
* The new test will be replacing the TULE test.
* The supplementary year courses will still end with the course participant being qualified to take the AT internship and test.

Essentially, there will be two paths of very different length:
* Pass the new test --> 6 months internship --> Swedish medical license
* Take a supplementary year --> wait for AT opportunity --> take AT internship for 18+ months --> pass the AT test --> Swedish medical license

(for the time being - this may change in the future)

Thanks for the website. What about the specialists educated abroad? Do you know they have to take the new test or as before they just have to work in probationary period to get the medical license?
